History

Established in March 2001, Wong Cho Tong Social Service Building is named after benevolent Mr. Wong Cho-tong in appreciation of his generous donation of $16.5 million towards the Building's capital cost. Since its inception, the Building has been championing the notion of service integration and providing diversified and value-added social services to the community. With the seamless collaboration of the service units housed in Building, Wong Cho Tong Integrated Home Care Service offers the public the convenience of community services and achieves the Continuum of Care.

Objective

  • To provide home care services, nursing and rehabilitation services to cater to the daily needs of elders who lack self-care ability, disabled persons and families in need.
  • To actualize the concepts of "ageing in place" and "continuum of care" in the community through the provision of a comprehensive range of home care and support service.

Service Description

To meet the varied needs of service users, Integrated Home Care Services provide services to the following three categories of cases:

  • Frail Case

  • Elders assessed to be in state of moderate or severe level of impairment by the Standardised Care Need Assessment Mechanism for Elderly Services, and in need of a comprehensive package of services.
  • Ordinary Case

  • Those service users, who suffer from no to mild level of impairment or disability, or those with moderate to severe level of impairment but require only personal care, simple nursing care and/or other support services.
  • Mild Impairment Case

  • Elders assessed to be at least with mild impairment and waitlisted for Integrated Home Care Services (Ordinary Cases).

Service Recipients

  • Elders aged 60 or above who lacks self-care ability and is living our service boundary
  • People with disabilities
  • Individuals or families with social needs
